To retire in São Paulo, most foreigners use Brazil's retiree visa, which asks for proof you can transfer at least US$2,000 a month from a pension. A retired couple renting in a middle-priced area spends about R$8,950 to R$15,680 a month (US$1,794 to US$3,142). That includes health plans priced for adults under 44. Plans cost more after 59, at a median R$1,496 per person in June 2026.

Is São Paulo good for retirees? Pros and cons
If you plan to retire in São Paulo, the draws are accredited private hospitals, free transit from 65 and nonstop flights home. The costs to plan for are rent and private health cover after 59.11 12 23 48 70
- Pro, hospitals. Einstein, Sírio-Libanês, Oswaldo Cruz and HCor hold Joint Commission International accreditation.
- Pro, senior benefits. Buses and the Metrô are free from 65, and people 60 and over get at least 50% off cultural, sports and leisure tickets.
- Pro, flights home. In August 2026, Guarulhos had about four nonstops a day to Miami, four to New York, two to London and five to Madrid.
- Con, health plans. Individual plans at 59 and over cost a median R$1,496 per person in June 2026. Only 25 insurers priced them in the city.
- Con, rent. São Paulo had the highest FipeZap rent per m² of the 22 state capitals in August 2026.
- Con, air quality. The metro region broke the state ozone standard on 65 days in 2024, mostly from August to October.
- Con, pensions. Foreign pensions are taxable once you're resident, and UK State Pensions are frozen.
How to get a retirement visa for Brazil
Retiring in Brazil as a foreigner usually starts with the retiree visa under CNIg Resolution 40/2019. It requires a retirement or survivor pension that lets you transfer at least US$2,000 a month to Brazil. Other regular income can top it up. The Ministry of Justice's list showed no later rule in October 2026.1

Applying from inside Brazil
If you're already in Brazil, you can ask the Ministry of Justice for the same residence with the same income proof. Since Resolution 51/2025, a visit visa can be converted through the MigranteWeb system. The fee is R$168.13 (about US$34), with a 30-day target for complete files. Once approval is published, you have 30 days to register with the Federal Police.1 3 5 9
After you arrive
You then register for the CRNM, the migrant ID card, which costs R$204.77 (about US$41). Until it arrives, the registration protocol and your passport give you the same rights for up to 180 days. With indefinite residence the card is valid for nine years, and it doesn't expire if you turn 60 before that date. More than two years abroad without justification can cost you your residence. A spouse or partner, children and parents can apply for a family reunion visa.6 7 8 9
Retiring in Brazil without a pension
Savings alone don't qualify for the retiree visa. The nearest option for a São Paulo buyer is real estate investor residence, which needs urban property worth at least R$1,000,000 (about US$200,000). The first R$1,000,000 must be your own money. Residence lasts four years and can become indefinite if you keep the property.4

A retired couple renting 50 to 70 m² in a middle-priced area such as Vila Mariana or Perdizes spends about R$8,950 to R$15,680 a month. Can you retire in São Paulo on $2,000 a month?
It's tight for a couple. The US$2,000 visa minimum, about R$9,980, sits near the low end of the middle-area range. Swap in two over-59 plans at the median R$1,496 each and that low end rises to about R$10,800, above the minimum. In lower-rent Santana or Vila Andrade, the range is R$7,710 to R$13,020 (US$1,545 to US$2,609), and the low end with over-59 plans is about R$9,560.10 11 12

Health insurance for retirees in Brazil
No private plan can refuse you because of your age. On plans sold since 2004, age-based increases stop at a final band starting at 59. From 60, the Older Persons Statute bans age pricing. Individual plan increases were capped at 5.11% for May 2026 to April 2027.13 14 15 18

Individual plans are scarce. In June 2026, only 169 of 4,684 medical plans priced for São Paulo city were individual or family plans. Amil, Bradesco Saúde and SulAmérica sold none. Waiting periods run up to 180 days, or 24 hours for emergencies. For a declared pre-existing condition, insurers can suspend surgery, ICU and complex care for up to 24 months.12 16 17 71
Medicare in Brazil
Medicare usually doesn't pay for care in Brazil. Most Medigap plans cover 80% of emergency care in the first 60 days of a trip, after a US$250 deductible, up to US$50,000 lifetime. Part B costs US$202.90 a month in 2026. Drop it and re-enroll without a Special Enrollment Period, and you pay 10% more for each full 12 months you could have had it but didn't. Check with Medicare before you cancel.20 21 22
Hospitals, public care and medicines
Einstein in Morumbi, Sírio-Libanês in Bela Vista, and Oswaldo Cruz and HCor in Paraíso hold Joint Commission International accreditation. Facilities expect payment at the time of care. At its Bela Vista unit, Sírio-Libanês lists cataract surgery at R$9,531 (about US$1,910) before doctors' fees.23 24 25 26
Legal residents can get an SUS card for public care. Farmácia Popular gives free medicines for blood pressure, diabetes, cholesterol, osteoporosis and more with a CPF, ID and any doctor's prescription. Care homes
Care homes for people 60 and over are called ILPIs. They follow Anvisa's RDC 502/2021, which sets one caregiver per 20 independent residents and one per 6 who need full care. Health plans can exclude these stays, so they're normally paid out of pocket. In August 2026 the city ran 20 public care homes for older people in vulnerable situations.19 29 30
Receiving Social Security and other pensions in Brazil
American retirees in Brazil can receive Social Security here, but Medicare usually doesn't pay for care in Brazil. If Social Security is your main income, the retiree visa still asks for proof of a US$2,000 monthly transfer, which other regular income can top up.1 20 31
Receiving Social Security in Brazil
Brazil is on SSA's International Direct Deposit list, so benefits can go into a Brazilian bank. A totalization agreement in force since October 1, 2018 combines US and Brazilian credits for old-age, survivor and disability benefits. You need at least six US credits, and it doesn't cover Medicare or SSI.31 32 33 34
Canadian and UK pensions
For Canadians, an agreement in force since August 1, 2014 covers CPP and Old Age Security but not the Quebec Pension Plan. OAS is paid abroad only after 20 years of Canadian residence after age 18.35 36
A UK State Pension paid in Brazil is frozen, since Brazil isn't on the UK's list for annual increases. It arrives every 4 or 13 weeks with a 0.39% currency conversion charge.37 38
Converting foreign income into reais usually carries 0.38% IOF tax. For other pensions, ask the paying authority how it pays into Brazil.39
Brazil tax on foreign pensions
On a temporary visa, you become a tax resident on your 184th day in Brazil within 12 months. A pension from abroad is then taxable. You pay monthly through the carnê-leão, due by the last business day of the next month, and in the annual return.40 41

There's no US and Brazil income tax treaty. Even so, Brazil credits federal income tax paid in the US, the UK and Germany by reciprocity, within limits. A UK treaty signed in 2022 isn't in force. Canada, Portugal, Spain, France, Italy, the Netherlands, Sweden and Switzerland have treaties in force. Australia has none.40 43 44 45
Foreign interest and dividends pay a flat 15%. The extra R$1,903.98 monthly exemption at 65 (about US$382) lists only Brazilian payers.40 42
US citizens still file US returns, by June 15 when abroad, and an FBAR once foreign accounts top US$10,000. Since the result depends on your income mix, get advice from a tax professional who works in both countries.46 47
Senior benefits in São Paulo at 60 and over
Brazil's Older Persons Statute (Estatuto da Pessoa Idosa) protects everyone 60 and over. It gives priority service at public and private counters, with extra priority after 80, and at least 50% off cultural, sports and leisure tickets.18

The senior bus card, the Bilhete Único Especial da Pessoa Idosa, is for residents of the city or metro region. Bring a photo ID issued in the last ten years, such as your CRNM. Add your CPF if it isn't printed on the ID, and proof of address from the last six months. It's free, arrives within 30 days and renews when used. Routes are in getting around São Paulo.48
The Pinacoteca is free at 60 and over instead of R$40 (about US$8), and MASP charges R$42 instead of R$85. The city's civil police also run eight stations dedicated to protecting older people.52 53 54
Quiet neighborhoods for retirees in São Paulo
These six areas score well for quiet streets in our data and sit near a metro station, a park or a hospital. Compare them all in the São Paulo neighborhood guide.
Vila Mariana
Line 1 runs under the main street, and the district holds the Hospital São Paulo campus. Its Paraíso area has HCor and Oswaldo Cruz.
Perdizes
Parts are heritage-protected, which limits redevelopment. Sumaré station is three stops from Trianon-MASP with no change.
Higienópolis
One stop from Avenida Paulista on Line 4, with Hospital Samaritano in the neighborhood.
Moema
Ibirapuera Park, about 124 hectares (306 acres), lies inside the district, which has three Line 5 stations.
Jardins
Garden streets zoned for houses near Oscar Freire station on Line 4. Sírio-Libanês has a Jardins unit.
Alto de Pinheiros
Streets of houses beside the Villa-Lobos and Candido Portinari parks. Transit is thin, and QuintoAndar rents rose 46% in the year to July 2026.

Rents per m² are FipeZap asking rents for August 2026. Bela Vista has Sírio-Libanês and Trianon-MASP station but scores low for quiet. Check the crime picture on Is São Paulo safe? before you sign a lease.11 24
Climate, altitude and air quality
The main INMET weather station, Mirante de Santana, sits at about 785 meters (2,580 feet). That tempers the heat but causes no altitude problems for most people. Under its 1991 to 2020 normals, daily highs average 26.1°C (79°F), peaking at 29.0°C (84°F) in February, and July lows average 12.8°C (55°F). About 76 days a year reach 30°C (86°F), about 14 nights drop to 10°C (50°F) or below, and January brings 292 mm (11.5 inches) of rain.61
Air quality deserves attention at older ages. Ozone broke the state standard across the metro region on 65 days in 2024, mostly from August to October. CETESB says older people may get a dry cough and tiredness even at moderate levels. When winter humidity drops below 30%, the city advises skipping outdoor exercise. Staying active with Sesc, classes and culture
Sesc São Paulo has run a program for older people since 1963. Its free credential is for commerce, services and tourism workers and their dependents. Even so, many activities across its 26 city units are open to everyone.65 66 67
USP 60+ offers free University of São Paulo courses to people 60 and over, often alongside undergraduates. The city ran 98 social centers for older people in August 2026, and on Sundays five free Paulistar bus circuits link parks and cultural sites from 8 a.m. to 7 p.m. For trips farther out, see weekend trips from São Paulo.30 68 69
Retiring in São Paulo vs Florianópolis and Curitiba
Weighing the best places to retire in Brazil? Under INMET's 1991 to 2020 normals, Florianópolis averages 21.1°C (70°F) a year against São Paulo's 20.4°C (69°F). Its FipeZap rent was R$61.19 per m² in August 2026, against R$65.36 in São Paulo.11 62
